Domain 6: Interprofessional Partnerships

Descriptor: Intentional collaboration across professions and with care team members, patients, families, communities, and other stakeholders to optimize care, enhance the healthcare experience, and strengthen outcomes.

Contextual Statement: Professional partnerships that include interprofessional, intraprofessional, and paraprofessional partnerships, build on a consistent demonstration of core professional values (altruism, excellence, caring, ethics, respect, communication, and shared accountability) in the provision of team-based, person-centered care. Nursing knowledge and expertise uniquely contributes to the intentional work within teams and in concert with patient, family, and community preferences and goals. Interprofessional partnerships require a coordinated, integrated, and collaborative implementation of the unique knowledge, beliefs, and skills of the full team for the end purpose of optimized care delivery. Effective collaboration requires an understanding of team dynamics and an ability to work effectively in care-oriented teams. Leadership of the team varies depending on needs of the individual, community, population, and context of care.

Level 1: Entry-Level 6.1a Communicate the nurse’s roles and responsibilities clearly. Incorporate nursing’s role in collaborative care plans, ensuring clarity and understanding of nursing contributions in optimizing patient outcomes.
6.1b Use various communication tools and techniques effectively. Utilize standardized communication tools and techniques to convey patient information accurately and concisely during handoffs and team discussions. Adapt communication tools to the context and audience.
6.1c Elicit perspectives to inform person-centered care. Describe interprofessional team perspectives during care planning discussions. Incorporate team member’s perspectives to inform person-centered care decision-making.
6.1d Engage in active listening. Identify how individual cultural backgrounds, biases, and communication styles influence interactions within interprofessional teams and impact patient care. Implement inclusive communication strategies.
6.1e Provide constructive feedback. Document and share information using standardized communication tools concisely and respectfully within required timeframes. Contribute concise, accurate information in a professional manner to facilitate timely decision-making and continuity of care.
6.1f Demonstrate cultural sensitivity in communication. Identify relevant legal, regulatory, and policy guidelines for interprofessional communication. Incorporate legal and policy guidelines into clinical communication.
       
Level 2: Advanced-Level 6.1g Document communication accurately. Analyze the strengths and limitations of interprofessional communication tools and techniques within specific team-based scenarios. Systematically assess and refine interprofessional communication through data-driven evaluation, incorporating stakeholder feedback and evidence-based practices.
6.1h Facilitate team communication. Use basic protocols to enhance clarity and reduce errors in interprofessional information exchange. Evaluate the effectiveness of existing interprofessional communication and take necessary action. 
6.1i Promote psychological safety in communication. Apply basic inclusive communication strategies within interprofessional teams. Evaluate and refine communication practices to promote a culture of respect in interprofessional teams.
6.1j Use plain language with patients. Apply nursing-specific knowledge when contributing to interprofessional care plans. Advocate for the integration of nursing’s unique disciplinary knowledge within interprofessional decision-making.
6.1k Advocate through communication. Use of evidence-based resources to inform consultations. Develop advanced practice guidelines or protocols to guide consultations and improve interprofessional outcomes.
